Dearly loved mum of Paula and her partner Phil,
loving nana of Leanne and Liam, and great nana of Ruby-Leigh and Scarlett,
Hazel will be dearly missed by all her family and friends.
A service to celebrate Hazel's life will be held at Woodlands Crematorium, Scunthorpe
on Friday 8th August at 11.20am.
Traditional black clothing preferred please.
